About Infinity Health
Infinity Health was founded in 2005 as a Federally Qualified Health Center in Decatur County, Iowa. Formerly known as Community Health Centers of Southern Iowa (CHCSI), the health care organization rebranded as Infinity Health in 2022 to reflect the overall reach and services it provides to the communities it serves in southern Iowa and northern Missouri. Infinity Health operates 11 locations providing a variety of primary care, behavioral health, substance use disorder, oral health and pharmacy services. The organization is also a member of the IowaHealth+ network.

History
In 1996, Healthcare Development of Lamoni was incorporated as a 501(c)(3) organization dedicated to recruiting new physicians, helping to ensure health care for those in need in Decatur County and surrounding areas, and building health care facilities. In 2004, the organization was awarded the 330 grant from the Department of Health and Human Services, and Community Health Centers of Southern Iowa (CHCSI) officially opened its doors for service on April 1, 2005.
On Jan. 1, 2022, the organization rebranded to Infinity Health to reflect the overall reach and services it provides to the communities it serves in southern Iowa and northern Missouri. Today, Infinity Health manages 11 locations, including primary care sites and centers offering behavioral health and substance abuse services.
