Infinity Health’s Sliding Fee Program

At Infinity Health, we believe that everyone deserves access to high-quality healthcare, regardless of their income level. That’s why we offer a Sliding Fee Program designed to make our medical, dental, and behavioral health services more affordable for you and your family.

What Is the Sliding Fee Program?

The Sliding Fee Program is a discount program for patients whose household income falls below 200% of the Federal Poverty Guidelines. It reduces the cost of medically necessary services, including provider visits, labs, procedures, and tests, based on your income and family size.

Whether you’re coming in for a wellness exam, dental checkup, or behavioral health support, this program helps reduce your out-of-pocket costs so you can prioritize your health without financial stress.

Who Can Apply?

Eligibility is based on total household income and the number of people in your household. “Family” includes all individuals living in your home who share expenses or are claimed as dependents. That could be your partner, your children, a grandparent, anyone contributing to or relying on the household’s finances.

  • If you live with roommates and cover your own expenses separately, you apply individually.
  • If your family shares expenses across generations, your income is combined.

What Services Are Covered?

Eligible services include:

  • Medical care
  • Behavioral health and substance use services
  • Dental services
  • Vision exams (provided by Infinity Health)

Services that are cosmetic, elective, experimental, or outside of Infinity Health are not covered. However, deductibles exceeding $1,000 may qualify.

Infinity Health’s Sliding Fee Schedule helps ensure affordable access to healthcare by adjusting service costs based on household income and size. Using the Federal Poverty Guidelines, patients are placed into one of four sliding scale categories (B through E), ranging from ≤100% to 200% of the poverty level. Each category determines the percentage of full service costs a patient will pay. For example, those in the lowest income bracket (Slide B) pay a flat fee for medical, behavioral, and substance use services, while higher brackets pay increasing percentages up to full cost. Dental services, mammography, and pharmacy fees are similarly scaled, with specific charges or percentage-based discounts applied. This system ensures that care remains accessible regardless of financial status.

How to Apply

It’s simple. Just complete the Sliding Fee Application and provide proof of income, such as:

  • Recent pay stubs
  • Previous year’s tax return
  • Social Security or unemployment benefits
  • Child support or alimony documentation
  • Self-employment income records

If your income changes throughout the year, let us know so we can update your eligibility. Once approved, your discount goes into effect retroactively, starting 30 days prior to the date your completed application was submitted.
