Meet the Providers Caring for Our Community
November 14th, 2025
At Infinity Health, our providers are more than medical professionals; they’re neighbors, mentors, listeners, and trusted partners in your health journey. Each one brings unique strengths, experiences, and heart to the care they deliver, especially in the rural communities we’re proud to serve.
We’re taking this opportunity to spotlight our dedicated providers who exemplify compassion, commitment, and excellence in everything they do.
Michael Black, NP
Michael Black, NP, joined Infinity Health in 2021, continuing a lifelong commitment to serving his hometown community of Lamoni. His path into healthcare began as a first responder, where he discovered how meaningful it was to help people feel better during their most vulnerable moments. After completing his training at SWCC and Graceland University, Michael built his care philosophy around giving patients the time and space to genuinely share their concerns.
He loves the close-knit nature of rural healthcare, providing care close to home for the people who helped shape his own story. When he’s not in the clinic, Michael enjoys fishing, woodworking, and spending quality time with his wife and their two dogs. A proud Chiefs and Royals fan, he jokes that if he weren’t a provider, he’d be in his workshop full time, crafting something new.

Andrew Sullivan, PA-C
Since 2015, Leon native Andrew Sullivan, PA-C, has been caring for the very community that helped raise him. With degrees from Mercy College of Health Sciences and Des Moines University, Andrew chose family and general practice because it brings healthcare back to what matters most; helping people feel better and improving their quality of life.
Andrew appreciates the unique impact rural providers can make where outside resources may be limited. His approach to care is straightforward and patient-centered: understand what’s wrong and help people get back to feeling like themselves again. Outside of work, Andrew enjoys golfing, riding motorcycles, and exploring the outdoors.
Nate Allen, DO
Dr. Nate Allen joined Infinity Health in 2013 right out of residency, and he has been a steady, compassionate presence in our communities ever since. Growing up in Oskaloosa and later training at Des Moines University, Nate felt called to medicine as a practical way to help people and reflect kindness and compassion through his work.
For Nate, rural healthcare is deeply personal, built on relationships, trust, and walking alongside patients through both challenges and celebrations. He is grateful for teammates like his nurse, Darcie Moore, and for every opportunity to listen to a patient’s story and care for the whole person, not just their symptoms.
Outside the clinic, Nate enjoys life with his wife and four children. Whether camping, working on projects, or spending time with their Living Hope Church family, he finds joy in being outdoors and staying connected to those he loves. A self-described “terrible beekeeper,” Nate always finds something new to learn, and approaches both hobbies and medicine with the same curiosity and heart.

Elizabeth Brant, ARNP
Elizabeth Brant, ARNP, has been on a healthcare path since age 12, when she began accompanying her grandparents to appointments and found herself wanting to understand medicine so she could help them navigate it. She became a CNA at 15 and has since built a 25-year nursing career marked by curiosity, compassion, and a love for caring for all ages, especially when it comes to solving urgent-care “mysteries.” A Leon native, Elizabeth’s education includes nursing degrees from Southwestern Community College, a BSN from the University of Iowa, and a Master of Science in Nursing from Clarkson College. She is certified through the American Academy of Nurse Practitioners and is also a Board Certified Emergency Nurse.
Before joining Infinity Health in 2022, Elizabeth spent 15 years in emergency and trauma medicine, serving in roles from staff nurse to educator to nursing supervisor in a large metro Emergency Department and Trauma Center. Her passion for acute care continues to shape her work as the Urgent Care provider in Leon. She also serves on the Emergency Nurses Association Iowa Council, where she has been active for more than a decade and currently holds the role of President-Elect.
Outside the clinic, Elizabeth and her husband, Travis, enjoy life on their small central Iowa farm with their English Springer Spaniels. They spend their free time gardening, hunting, fishing, raising honeybees and puppies, and cheering on the Iowa Hawkeyes football and wrestling. Go Hawks!
Jordan McCloney, ARNP
Since joining Infinity Health in 2021, Jordan McCloney has been committed to caring for her community in a deeply personal and meaningful way. A native of Osceola, Jordan has wanted to work in medicine since she was a child, driven by a desire to help people and solve problems. Though her path ultimately led her to nursing instead of medical school, she’s grateful to be in a role where she can make a real impact on patients’ lives every day. She loves seeing improvements in her patients’ health and well-being and celebrating every victory, big or small.
Jordan has a strong interest in women’s health and is currently pursuing HRT certification to better support her aging female patients. Her care philosophy centers on truly listening and understanding not just what symptoms someone is experiencing, but how those issues impact their daily life. She admits she’s “extremely stubborn” in the best way: she rarely gives up on finding a solution, and if she can’t solve the problem herself, she’ll make sure she finds someone who can.
Outside of work, Jordan describes herself as “a mom full-time and a nurse on the side.” She and her husband stay busy with their two kids and three dogs, and she loves spending time with her family, tackling home projects, cooking, baking, and diving into DIY projects with as much confidence as YouTube can give her.

Thank You to Our Providers
Infinity Health is stronger because of the people behind the care; providers who show up every day ready to listen, support, and make a difference in the lives of our patients. We’re grateful for the entire Infinity Health team for continuing to bring compassion and connection to our communities.
Stay tuned for more provider spotlights as we continue sharing the stories of the incredible people who care for you.
