Infinity Health Receives Patient Centered Medical Home Accreditation with Behavioral Health Distinction

Leon, Iowa – October 2023. Infinity Health’s Lamoni and Leon locations has achieved Patient Centered Medical Home (PCMH) accreditation with Behavioral Health Distinction from NCQA (National Committee for Quality Assurance)!

The PCMH model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. PCMH Distinction in Behavioral Health Integration distinguishes practices that provide integrated behavioral healthcare as a part of patient-centered medical home.

Infinity Health has been accredited with PCMH Level 3 since 2014. Annual renewals are completed showing that Infinity Health continues to invest in a model of care that puts patients at the forefront of care and makes quality improvement a priority. PCMH – Behavioral Health Distinction accreditation standards were released in 2022.
